Airline power is 15v DC. Car power is a nominal 12v, but can fluctuate and is also rather noisy.
The airline adapter Apple sell is simply a cable and it doesn’t regulate the power. 15v is enough to run a Macbook (but not charge it); 12v isn’t enough to do either.
To use a cars 12v you need a converter that boosts and regulates the fluctuating and noisy supply.
